Ifeanyi Ezeani

Dr. Ifeanyi Ezeani, DBA-HCM, MBA

Chief Executive Officer

Ifeanyi Ezeani is the founder and Chief Executive Officer of Compassion Pathway Behavioral Health LLC. Ifeanyi earned a Doctorate in Business Administration with an emphasis in Healthcare Management, an MBA with an emphasis in Healthcare Management, and a bachelor’s degree in Business Administration. He is currently finalizing his dissertation in Industrial-Organizational (IO) Psychology. Ifeanyi brings a wealth of experience as CEO at a mental health care organizational agency in Northern California and has extensive leadership and management experience in several leadership positions at CALPERs and FedEx Operations Management. Ifeanyi understands the urgent need for facilities and is passionate about serving clients with mental illness and their rehabilitation.

Ifeanyi has four children and enjoys spending time with them in his spare time. When he finds extra spare time, he attends cultural events and travels.

Safaratu Okolo

Safaratu Okolo, MSW

Chief of Programs

Saf received her Master of Social Work (MSW) in 2023 and received her B.A. in psychology. Saf has been with CPBH and Compassion Valley since we opened in 2020. Saf brings to Compassion Pathway Behavioral Health over 25-years of experience as lead teacher and administrator for pre-school programs. She has a passion for helping others and is a mental health advocate and mentor. Saf has been instrumental in providing quality care for our clients.

In her free time, Saf enjoys spending time with her three children and her parents, learning new recipes, and studying.

Michelle Pontillas Frase

Michelle Frase, BS

Chief of Staff

Michelle joined CPBH in 2022. She has a Bachelor of Science degree in Business Administration from the University of Phoenix and has over 20 years of diverse administrative experience ranging from the high-tech industry, education, and non-profits to serving American Indian and Alaska Native communities. She is enthusiastic about supporting CPBH leadership and staff. She is a champion in managing the main office, answering calls, and providing administrative support for the CEO and operations. Her goal is to keep things running smoothly like a well-oiled machine. Michelle loves to help people and is grateful for the opportunity to support our clients.

In her spare time, she enjoys spending time with her five children, playing with their dog, Rocco, and traveling as often as possible.

Jalia Kizito

Jalia Kizito

DNP, FNP, PMHNP-BC

Jalia is a doctoral prepared Psychiatric Mental Health Nurse Practitioner (PMHNP) with more than two decades of combined healthcare experience. She earned her Master of Social Work from CSU Sacramento in 2013, followed by a Master of Science in Nursing (FNP) in 2017 and a Doctorate in Nursing Practice in 2018 from Samuel Merritt University. She later completed her PMHNP Certificate at UC San Francisco in 2019.

Her diverse background includes roles in social work, nursing, and case management, as well as serving as a Clinical Instructor at the UC Davis School of Nursing—experience that has shaped her into a well-rounded clinician. Jalia has also worked extensively as a PMHNP, providing psychiatric care to individuals of all ages, from children as young as seven to adults, across community-based programs and private practice settings.

She brings to our team a deep understanding of integrated behavioral health, strong clinical expertise, and an unwavering commitment to compassionate, evidence-based mental health care.

Shana Doolin

Shana Doolin

DNP, FNP-BC, PMHNP

Shana Doolin is our Clinician for Compassion Pathway Behavioral Health for our Northern Region facilities. A Doctor of Nursing Practice and board-certified Family Nurse Practitioner, Shana brings over 30 years of diverse healthcare experience. She earned her nursing degrees at Pacific Union College, California State University Sacramento, and Holy Names University, and completed her DNP at Brandman University in 2017. Shana has worked in family practice, correctional forensics, aerospace medicine, and Indian Health Services, and she is currently pursuing a Psychiatric Mental Health Nurse Practitioner credential.

Passionate about serving her community, Shana volunteers at free healthcare clinics and leads a diabetic support group. A licensed pilot since 17, she participates in Angel Flight West, flying patients to medical appointments. Shana also enjoys travel, mountain biking, scuba diving, theater, and cheering for her favorite sports teams—the LA Dodgers, SF 49ers, San Jose Sharks, and GS Warriors.

Dia Yang

Dia Yang, LCSW

Therapist/Clinical Supervisor

Dia received her Bachelors in Psychology from U.C. Davis in 2001 and graduated from Chico State School of Social Work in 2013. Dia worked with at-risk youth and ethnically diverse populations for several years. She then provided mental health services in County Mental Health for over eight years before joining Compassion Pathway Behavioral Health. Dia currently provides teletherapy for clients in her own practice and mental health services, clinical support, and supervision for therapist interns as part of the Compassion Pathway Behavioral Health/Bella Vida team.

Dia believes mental health wellness and balance play an important role in overall wellness. When not in the office, Dia enjoys spending quality time with her children, hiking the outdoors with her family, reading, and listening to relaxing music.

Danielle Bogan

Danielle Bogan, LMFT

Licensed Marriage & Family Therapist

Danielle recently joined CPBH in 2025. Danielle received her bachelor’s in human services from the University of Phoenix in 2013 and went on to obtain her Master of Science in Counseling Marriage and Family Therapy in 2016. Danielle has over 13 years of experience working for both non-profit and county entities serving youth and adults in the family court systems as well as Behavioral Health. Danielle worked for County Behavioral Health for 9 years as a clinician, supervisor, and manager. Danielle currently provides individual therapy and intensive outpatient services for a private, not-for-profit organization. In addition to providing clinical support and supervision for therapist interns at Compassion Pathways Behavioral Health. Danielle is dedicated to providing the highest quality of care and compassion to her clients.

In her spare time, Danielle enjoys spending time with her husband, 2 children, 3 cats, and 2 bloodhounds. Danielle enjoys reading, dancing, and gardening.

Rachel Breedlove

Rachel Breedlove

Regional Director, Greater Sacramento / Chief of Compliance

Rachel Breedlove is the Regional Director for the Sierra region at Compassion Pathway Behavioral Health. She oversees operations at Cirby Ranch, Garfield Wellness & Recovery Center, Compassion Springs SRF, and several upcoming facilities. Rachel joined Compassion Pathway Behavioral Health in May 2022 as a Mental Health Worker at Cirby Ranch and quickly demonstrated her dedication to providing exceptional care and leadership. With over 20 years of experience in the mental health field and administration, including Human Resources and client care, Rachel brings a wealth of knowledge and a compassionate approach to her role. Her background includes extensive experience in supporting individuals with diverse needs, ranging from mental health and dual diagnoses to aging and physical disabilities. Rachel’s dedication to the mental health community, combined with her extensive experience and compassionate approach, makes her an invaluable member of the Compassion Pathway Behavioral Health team.

Outside of her professional responsibilities, Rachel enjoys spending time with her family, engaging in creative pursuits like art and gardening, practicing yoga, and exploring the outdoors. She is also a proud dog lover and enjoys volunteering as a Girl Scout Troop Mom, where she serves as an archery instructor and First Aid Trail Guide.

Simonie Webster

Simonie Webster, LPT

Regional Director, Sierra / Chief of Crisis Services

Simonie plays a pivotal role as our Regional Director for the Shasta-Cascades region, leading and managing our team of employees to achieve CPBH’s organizational goals and objectives. She oversees the Bella Vida Center, Anderson Creek SRF, Monte Vista ARF, and our upcoming facilities.

Simonie joined Compassion Pathway Behavioral Health in August 2023 as the Licensed Psychiatric Technician at our Bella Vida Center. With over 20 years of experience in the mental health field, she began her career working with foster youth, later transitioning to the adult side and gaining valuable experience in several Psychiatric Health Facilities. Simonie also managed Mental Health services for an area hospital.

Simonie has found her niche working with individuals in crisis who have mental illness and/or dual diagnosis. Through an empathetic, open, and non-judgmental approach, she builds on client strengths to empower them to meet personal goals and overcome barriers. Simonie is LGBTQ-affirming and works from a trauma-informed perspective.

Outside of work, Simonie enjoys spending time with her four adult children and three furry friends.

Uzorchi Alex

Medication Services Manager

Uzorchi Alex Anugwom is a dedicated and skilled Registered Nurse with a robust background in providing high-quality patient care. A proud graduate of Arizona State University with a Bachelor of Nursing, Uzorchi has demonstrated exceptional clinical expertise and a commitment to patient care throughout their career. In their current role, Uzorchi is responsible for supervising a team of nurses, facilitating effective communication, and ensuring that care protocols are adhered to. They also lead educational initiatives, empowering patients with comprehensive knowledge about their medications, potential side effects, and the importance of adherence to treatment plans. This role involves not only clinical oversight but also a commitment to enhancing patient understanding and engagement in their own mental health care. Uzorchi excels in overseeing nursing staff and providing critical education on medication management to patients. With a strong foundation in both clinical practice and patient education, she plays a pivotal role in ensuring the highest standards of care and support within the facility. Uzorchi plans to further her education by obtaining a doctoral degree in Psychiatry Mental Health.

Outside of work, Uzorchi enjoys spending time with family and friends or participating in cultural events.
